STEPS TO SUCCESS--TRANSITION AT AGE 3: DVD EVALUATION

IMPORTANT: WE WOULD APPRECIATE YOUR PARTICIPATION - PLEASE COMPLETE AFTER VIEWING DVD

PART I. Instructions: The intent of this DVD is to help parents understand transition timelines and activities as their children near age three and transition from First Steps Early Intervention to Early Childhood Special Education, Head Start Programs, or other options. Parents need to be aware of transition planning activities that will occur. The DVD is a tool to assist families and service providers to plan together so that children and families experience a smooth and effective transition.

This DVD was funded by the Individuals with Disabilities Education Improvement Act (IDEA) General Supervision and Enhancement Grant. Indiana must report on the effectiveness of this DVD. We would appreciate your letting us know what you think. Information is confidential and anonymous.
